The Billionaire Ex's Secret Twins

Before Kale's surgery, Simone tells him his mother has finally accepted her and their baby. Once Kale is wheeled in, his mother forces Simone to disappear with the child. To ensure he gets the best treatment, Simone lies to Kale that she has found someone richer and no longer wants him, then goes into labor as she leaves.

Simone gives birth to a boy, but Kale's mother takes the baby and threatens her. Five years later, she raises her daughter Elsa alone, working multiple jobs. At school, Elsa clashes with Peregrine, and the other parent demands expulsion. Simone arrives and begs for leniency.

Kale storms in demanding to know who hit his son Peregrine. Peregrine claims Elsa beat him up, and the other parent backs him up. Kale insists on expulsion, but Simone speaks up to stop him, and he recognizes her voice.

Simone tries to leave with Elsa, but Kale blocks her, mocking her for marrying richer yet living poorly. When Elsa defends her mother, Kale hears her name and demands to know if Elsa is his daughter. A flashback of his mother's threat stops Simone from telling the truth.

Kale demands Elsa's age; Simone lies that she is five and her husband's child. Elsa corrects him—she is six—and Kale concludes Simone left him for money. Simone slaps herself in apology, begging him to let her daughter stay in school. Peregrine softens and asks to go home, but Kale warns this is only the beginning.

The doctor tells Simone that Elsa has a congenital heart condition and missed the optimal surgery window. Only a research hospital under the Thorne group might save her. Simone rushes to Kale's hospital room and begs him to admit her daughter. Onlookers recognize her as the woman Kale's mother drove away—Peregrine's real mother.

Kale refuses to acknowledge Elsa as his daughter, calling her another man's child. His mother Gail blames Simone for abandoning him on the day of his surgery. Simone offers to take her daughter and leave the city forever, but Kale wants her to feel his past pain and turns her away.

Simone rushes Elsa into the hospital with no pulse, but the doctors refuse to treat her on Kale's orders. When Kale learns Elsa has the same congenital heart condition as his own, he suspects she might be his daughter and races to the hospital.
